About Your First Office Visit

Prior to your first appointment, you will need to download your new patient intake forms, complete them, and mail them to the address below. The questionnaires are used to help identify areas where you may have health problems or risks. In addition, Dr. Miller will gain a comprehensive picture of your health status and will be able to focus on the health issues that concern you most.

Your first office appointment is 1.5 hours usually consisting of a detailed interview, a general physical exam, and recommendations for an individualized care plan. if your particular case is more complex, the doctor may recommend deferring the physical exam to a later appointment to allow time for a more comprehensive interview.
